Employees’ State Insurance (ESI) is a self-financing social security and health insurance scheme for Indian workers. ESI is a social security scheme like EPF. It is an insurance scheme that would protect the interest of workers in possibilities such as sickness, maternity, temporary or permanent physical disablement and death due to employment injury resulting in loss of wages or earning capacity. The Act also guarantees reasonably good medical care to workers and their immediate dependents.

Registration under ESI is compulsory:
– For every non-seasonal factories, shops, hotels, restaurants, cinemas, road-motor transport undertakings, newspaper establishments, establishments engaged in Insurance Business, Non Banking Financial Companies, Port Trust, Airport Authorities and Warehousing establishments, Private Medical Institutions, Educational institutions and to contract and casual employees of Municipal Corporation/Municipal Bodies employing 10* or more persons.
*Note: However the threshold for Coverage of establishments is still 20 Employees in Maharashtra and Chandigarh.
– For every employee who is getting less than INR 21,000/- per month [Rs. 25,000/- per month in the case of persons with disability].
1. Copy of the Pan Card of the Company/Firm/LLP/Society/Trust.
2. Self attested copy of, Bank Statement, Landline Bill, Power Connection Bill, water Bill in the name of the establishment, Any license/certificate/number issued by any Govt. authority etc., as address proof of the establishment.
3. In case of company Memorandum/Articles of Association/bi-laws of Society or Trust/ Partnership Deed
4. ID, Address Proof, Mobile Number & E mail ID of all Directors/Partners/Members of Society or Trust.
5. Copy of registration certificate issued under Companies Act, GST, Co-operative Society act, Importer and Exporter code etc.,
6. In case of factory, License number and date of issue of license.
7. Business Activity of the Company/Firm/LLP/Society/Trust.
8. Details of other Registration in any other statute, if any.
9. Details of employees along with their address, Basic Salary and date of joining the firm/company with .
10. E-Mail id and Mobile number of the firm/company.
11. Details of Bank account along with a Cancelled cheque.
12. First Sale & Purchase Bill.
13. Family Photo in Duplicate of employees.
